Supported by HealthTexas Provider Network (HTPN) and Baylor Scott & White Health, the community care program designs, implements and operates innovative strategies that increase access to health services, provide quality care and improve health outcomes for medically underserved populations. The community care strategy includes a network of HTPN-operated or managed primary care clinics and supporting programs which target underserved patients from the community and hospitals following discharge. Today, we operate nine community care clinics across the Dallas-Fort Worth Metroplex, including:

- Baylor Scott & White Community Care – Fort Worth
- Baylor Scott & White Community Care – Worth Street
- Baylor Scott & White Community Clinic – Irving
- Baylor Scott & White Community Clinic – St. Philip's
- Baylor Scott & White Douglass Community Clinic – Plano
- Baylor Scott & White Health and Wellness Center at the Juanita J. Craft Recreation Center
- Hope Clinic of Garland
More than 14 full-time equivalent (FTE) physicians and nurse practitioners provide care to nearly 8,500 underserved patients at these locations. Seven of the nine clinics have achieved National Committee for Quality Assurance certification as a Patient-Centered Medical Home and all community care practices participate in HTPNs quality improvement efforts around preventive health services, diabetes management and patient satisfaction.
By implementing innovative support programs for patients and providers, our community care program achieves quality outcomes and improves care coordination within underserved populations. A team of community care navigators (specially trained community health workers) are located at four hospital campuses and work to ensure patients successfully transition from hospital to medical home. Another group of trained community health workers, known as diabetes health promoters, provide one-on-one diabetes education to uninsured persons with diabetes who have historically had difficulty accessing educational services.
Central Texas
Washington County Community Clinic
Baylor Scott & White Medical Center – Brenham collaborates with Washington County and Faith Mission of Brenham to improve access and provide services to the low-income and medically underserved residents of Washington County. This is done through significant in kind support of resources (supplies, staff, labs, etc.) to the community clinic throughout the year.
Family Health Center FQHC at Santa Fe Clinic
The Community Clinic at Santa Fe in Temple and the MacArthur Pediatric Clinic in Waco are the result of a partnership between Baylor Scott & White Central Texas division and the Heart of Texas Community Health Center. These clinics are nonprofit, Federally Qualified Health Centers that deliver medical care to the underserved citizens in Bell and McLennan County. In addition to their partnership with Baylor Scott & White in Central Texas, HOT CHC also operates 12 additional FQHC sites in McLennan County.
The Community Clinic at Santa Fe serves uninsured patients as well as patients with service through Bell County Indigent Health Services, and uses a sliding scale to adjust their fee-for-service.
